Neuropeptides are peptide neurotransmitters that have the capability to interact with the peripheral and central nervous systems, producing physiological responses via a number of mechanisms. An increasing number of neuropeptides have been identified in the human heart and blood vessels, in both healthy and diseased states. There is much evidence that they are involved in pathophysiological conditions associated with cardiovascular disease, including atherosclerosis, hypertension, cardiac arrhythmias, and myocardial ischaemia. The endothelium is the source of nitric oxide and many of the endothelium-derived vasoactive neuropeptides are also produced by other tissues, including the heart, aorta, kidney, lung, adrenals, pancreas, gut and brain. Thus, peripheral neuropeptides released from the endothelium and peptidergic nerves can interact with each other and/or have access to nervous and endocrine neurones. The extent to which the endogenous peptides in the heart and circulation play a role in cardiovascular diseases in humans remains to be established. Recent experimental studies are providing evidence that cardiovascular responses to exogenous neuropeptides are present in humans. However, little is known about the role of neuropeptides in clinical conditions.
